Medication Procedure:
According to New York State Law, there are certain criteria to be met before a child can be given medication in school.
Medication may not be administered unless written permission is obtained from the parent. Written permission, as well as directions regarding frequency, dosage and possible side effects/adverse reactions, must also be obtained from the doctor. If your physician does not have the “ New York State Medication Authorization for Administering Medication in School” form, they are available in the Health Office or on-line.
NO medication is to be sent in with students or kept in the classroom. An ADULT must deliver any medication in the original labeled container from the pharmacy.
