Immunization

New York State Public Health Law, Section 2164 mandates that schools shall not permit a child to be admitted unless the parent provides the school with an immunization certificate or proof from a physician, nurse practitioner or physician's assistant that the child is in the process of receiving the required immunizations.


Please check with your health care provider as soon as possible to make sure that your child has all the needed immunizations.


Required Immunizations:

  • Polio: 3-4 doses
  • Hepatitis B: 3-4 doses
  • Diphtheria/Tetanus/Pertussis: 3-5 doses
  • Measles/Mumps/Rubella: 1-2 doses
  • Varicella (Chicken Pox): 1-2 doses
  • Tdap: 1 dose before age 11

Your child's attendance may be disrupted if the health office has not received documentation of all required immunizations prior to the first day of school.

All the above immunizations must be documented by your health care provider, health department, or official copy of the immunization record from the child's previous school (a copy of the original immunization record from the healthcare provider-not a copy of the school health record) is acceptable. A NYSIIS/NYCIR record is also acceptable. The exact date each immunization was given must be included in the record.
